Requirements for the application for BS:

  1. PRC Certificate of Sea Service (with company dry seal, signature of ship owner or management,
    with seal of the vessel, signature of Master or Chief Engr. and duly notarized)
  2. Company Certification (with company dry seal and signature of ship owner or manning agent)
  3. Transcript of Records (completion of academic requirements)
  4. TRB/ISF Logbook (properly filled-up and met the minimum standard requirements)
  5. Crewlist (for domestic vessels)
  6. Documentary stamps
  7. Photocopy of SIRB/Passport
